During a routine inspection

We spoke with two people who were using the service who told us that staff involved them in making decisions. We were told that staff sought people's consent before supporting them. On the day of our inspection we saw that staff asked if people wished to carry out an activity beforehand. People's choices were respected.

We spoke with two people who were using the service to ask if they were satisfied with the care and support they received. One person said, 'Things are ok at the moment.' We were also told, 'I am happy here, I can come and go and the staff are OK.'

We spoke with the three members of staff who were working on the day of our inspection. The staff we spoke with felt that the home was kept hygienically clean and they had no concerns in this area. We saw staff carrying out various cleaning duties on the day of our inspection such as mopping floors.

The manager was completing audits in areas such as the safety of the environment and the completion of records. Where areas for improvement had been required these had been carried out. Since our previous inspection a new kitchen had been installed as this had been identified as an area of the home that required improvement.

During an inspection looking at part of the service

We spoke with two people who were using the service. The people we spoke with told us told us they had been involved in producing new care plans. People had also been involved in choosing activities they would like to take part in and planning a new menu.

We asked people if they felt staff treated them with dignity and respect and were told, 'Staff listen to me and they are polite. I get on better with staff now.' We were also told, 'I like it here and I like the staff.'

The people we spoke with told us they felt safe living at the home and that they felt things had improved recently.

The provider told us they would carry out maintenance and improvements to the house to address the areas for improvement we identified in our previous visit. We saw that some of these had been addressed, for example there was a new system in place of colour coded mops for cleaning different areas. An exposed wire had been covered over in a bathroom.

However some other areas for improvement had not been addressed. For example no work had been carried out to improve the kitchen area. We saw that one work surface was damaged and there was a gap in the flooring which had collected dirt. The acting manager told us they were in the process of obtaining quotes for a new kitchen but we did not see evidence of this.

During a routine inspection

People living at the service were involved in decisions but only to a limited extent. One person said: 'I get on with the staff. They do ask what we think some of the time.' Advocates were not accessed to support people living at the service. One person said: 'I've heard of advocates but not done it.' One person that we spoke with told us that snacks were available. However on the day of the inspection we found concerns with the availability of snacks.

Two people that we spoke with told us that two members of staff speak to them in a very disrespectful and threatening manner. This matter was reported to the manager immediately. One person we spoke with was unsure if there was a plan of care and said they had not seen it. Another person we spoke with said the staff looked after them ok.

We spoke with people living at the service about how safe and supported they felt. Two people we spoke with told us that they were being bullied by each other. Another person told us that due to the concerns that they had with how two members of staff treated them they only felt safe some of the time.

One person that we spoke with told us: 'I think the staff are ok and they seem to be trained.' One person raised concerns about staff administering medication, they told us: 'Some staff cannot do meds; other staff have to come in to do them.' We asked one person about their experience of giving feedback about the quality of services provided, they told us: 'I do not get asked to complete any questionnaires.'
