Records did not show that people had agreed to the support they were receiving. One relative told us they had not been involved in planning care planning and one person using the service confirmed this.
We saw that care plans were not personalised to individual's needs and not kept up to date. Records showed that people were not always receiving the support they were contracted for. One person told us their support worker was good and they were happy with the care they received. '
Systems were not always in place to protect people from harm as risk assessments and plans to minimise the risks were not in place. Financial and recruitment procedures were not robust enough to protect people from potential abuse. Staff were not given training and supervision to ensure good care and support was provided.
There were no systems in place for auditing procedures and monitoring the quality of the service provided. Relatives told us they were not always happy with the response they received when they raised concerns.
Records were not well maintained and not easily accessible.
