During a routine inspection

Tariro House is a locked ward in the community. People using service participated in planned rehabilitation with the aim of resettlement in the community. At the time of our inspection there were 11 people living at Tariro House. All the people using the service were forensic patients; these are people who overlap the criminal justice and mental health systems. We spoke with seven people using the service and five members of staff including the registered manager.

People told us they were happy at Tariro House. Most people said they would recommend it to others as it was a good rehabilitation unit. One person said, 'I had no choice in coming here but I'm glad I did. It helped me change my lifestyle.'

All the people we spoke with told us they had given their consent to care and treatment they were receiving. People's consent was regularly reviewed.

There were suitable arrangements in place to ensure people using the service were safeguarded against the risks of abuse.

Staff told us they had good support from their managers and the rest of their team. They were able to access training relevant to their role.

Records showed that the provider monitored the quality of the service by auditing records, investigating complaints and incidents and obtaining feedback from people using the service and staff.

During a routine inspection

We spoke with five of the fifteen people using the service at Tariro House.

Most people we spoke with told us that they felt involved in decisions about their care. They had access to their care plans and were involved in developing it.

People told us they felt well looked after and supported by the staff team.

People told us they felt safe, and did not experience any bullying or harassment in Tariro House.

Tariro House was suitably staffed at the time of our inspection.

The provider took incidents seriously, investigated and learned from them. However, we found that the Care Quality Commission had not been notified of some incidents relating to people using services.

Mental Health Act Commissioner reports

Each year, we visit all NHS trusts and independent providers who care for people whose rights are restricted under the Mental Health Act to monitor the care they provide and check that patients' rights are met. Immediate concerns raised by patients on those visits are discussed, if appropriate, with hospital staff.

Our Mental Health Act Commissioners may carry out a number of visits to each provider over a 12-month period, during which they talk to detained patients, staff and managers about how services are provided. In the past, we summarised themes from the visits and published an annual statement followed by the provider's response where applicable. We are looking at different ways to indicate the outcomes of our monitoring in the future.
