During a routine inspection

We used a number of different methods to help us understand the experiences of people using the service, because the people using the service had complex needs which meant they were not always able to tell us about their experiences. We spoke with a relative of a person using the service who told us that the home kept them updated on their relative's care and that another member of their family had been involved in their relative's care planning. They said that ''all the staff are very good, excellent and engaged''.

People were given appropriate information and support regarding their care and treatment. Health Action Plans (HAP) and Person Centred Plans (PCP) were in easy read accessible formats so that people understood them. Staff would verbally go through the plans with people.

Care plans and assessments of people's capabilities were carried out annually and updated in between if a person's needs changed. We found that the home was updating these documents in line with the provider's own guidance.

The provider had an effective system in place to identify, assess and manage risks to the health, safety and welfare of people using the service. For example Yarrow's head office carried out spot checks to ensure the home were meeting standards across a range of areas i.e. care planning. The provider also had arrangements in place to identify staff training needs and other areas staff needed support with.

During a routine inspection

We inspected five standards at the home and on the day of the inspection we found evidence that the provider was non compliant with one of these standards.

We used a number of different methods to help us understand the experiences of people using the service, because the people using the service had complex needs which meant they were not able to tell us their experiences. We spoke to the relatives of people who told us that they were regularly involved in peoples' care planning meetings and staff would discuss with them for example their relatives' behaviour and their likes and dislikes. They said the manager also kept them updated on their relatives' care outside of these meetings. One relative said ''I cannot tell you how fantastic staff are, they go above and beyond''.

On the day of the inspection we saw that interactions between people and staff were positive and that people were treated with dignity and respect. People appeared content in the company of staff. We saw that some people were allowed to leave the premises and staff checked they were okay and were wearing warm clothing. One person was reminded of what they were having for lunch by staff and they appeared to be happy with the food.

During a routine inspection

We spent time observing interactions between people who use the service and staff. We saw that staff understood individual's needs and acted on them. We saw people communicating non verbally and staff were aware of what the signs indicated. We saw staff interacting with people in a warm and respectful manner. People appeared happy, engaged and comfortable in the presence of staff, visitors and other people who use the service.
