During a routine inspection

We spoke with five staff, four visitors and ten people who used the service. Before people received any care or treatment they were asked for their consent and the provider acted in accordance with their wishes. One person who used the service told us: "The staff listen to my opinion." A member of staff told us: 'Gaining consent at every stage of delivering care is so important.'

Care and treatment was planned and delivered in a way that was intended to ensure people's safety and welfare. Comments we received were predominantly positive. People who used the service told us: 'It's a brilliant place and the staff are lovely.' 'It's a first class place. I couldn't be anywhere better' also 'It's a long day and there's not much going on.'

There were enough qualified, skilled and experienced staff to meet people's needs. One staff member told us: "It's always busy so we need to work together effectively.' One person who used the service told us: 'They (staff) always take time to chat with me.'

The provider had an effective system to regularly assess and monitor the quality of service that people received. The provider also had an effective system in place to identify, assess and manage risks to the health, safety and welfare of people who used the service and others.

People were protected from the risks of unsafe or inappropriate care and treatment because accurate and appropriate records were maintained.

During an inspection looking at part of the service

On this occasion we did not speak to people who used the service as we visited to review the progress in relation to improving care records. However their experiences were captured through checking records and considering other information we received from the provider. We also spoke to three members of staff.

During an inspection in response to concerns

People told us staff were very good, they always had time and did not rush. They also said staff were considerate and always gave choices.

We were told by people that staff listened to them and respected their choices and decisions.

One person we spoke with told us, 'I am happy with all the service provided, it has been a difficult time for me but staff have been very supportive'.

Another person told us, 'I am well looked after'.

During an inspection looking at part of the service

At the time of our visit a residents and relatives meeting was taking place and we attended part of the meeting. Some issues raised were regarding staffing numbers and the quality of the food. The acting manager and staff took on board what was said and agreed to look into it and let them know the outcomes. There were also a number of very positive comments made.

One person said, 'I would like it noting on the minutes that we have all seen a vast improvement over the last few months, the staff have worked very hard and things are going in the right direction.'

During an inspection looking at part of the service

People we spoke with told us that the staff maintained their privacy and dignity and they were respected. People also told us staff listened and acted on to their choices and wishes.

People also told us they had seen an improvement over the last few weeks due the changes occurring at the service.

People said they were not always able to input to their own care as staff were sometimes rushed.

People told us that they were able to take part in some activities although there were not always activities taking place.

One person told us the activity person tried to get people to take part in an activity that wasn't always appropriate. They also told us they used to do games that were exercise, like throwing a ball to each other. They said, 'That made us talk together and enjoy the activity, even people that didn't have capacity to understand enjoyed the interaction'.

People we spoke with told us that staff explained all procedures and treatment to them and respected their decisions about care, although sometimes it could be very rushed.

People told us that their interactions with most staff were positive and they were treated as individuals.

People we spoke with told us meetings used to take place but they had not had one for a while. They told us they liked it when residents meeting were arranged and hoped one would be organised.

During a routine inspection

People who used the service and their relatives told us that staff were friendly, supportive and discussed their care and treatment with them on a regular basis.

People told us they were encouraged to be as independent as possible; visitors to the home were made welcome by the staff and there was a good selection of activities to take part in.

Relatives told us they had some concerns with regard to staff not picking up prescriptions from the GP and pharmacy in a timely manner. Other concerns we were told about related to staff not changing bed linen on occasion and people using the service looking 'unkempt' at times.
