During a routine inspection

People told us they were satisfactorily cared for though they mainly spoke about what was happening to them 'in the moment'. They said, "I've had my breakfast and I am happy to stay here at the dining table for a while yet" and "I don't do much, but I like to read the paper."

We found that peoples' needs were assessed and care and treatment was planned and delivered in line with their individual care plan. We saw that care plans reflected needs and had been reviewed and that diary notes recorded care that had been given.

We found that people that used the service were protected from the risk of abuse because the provider had taken reasonable steps to identify the possibility of abuse and prevent abuse from happening.

Appropriate arrangements were in place in relation to obtaining, handling, storing, administering and disposing of medicines and we found that people received their medication when they needed it.

We found there were enough qualified, skilled and experienced staff on duty to meet peoples' needs, but that changes in the way the manager was deployed would have allowed them to work more intensely on management tasks.

We saw from documentation that people that used the service, their representatives and staff were asked for their views about their care and treatment and their views were acted on.

During an inspection looking at part of the service

We carried out this inspection visit to look at the improvements Wyton Abbey had achieved since our last full inspection in May 2012 and our combined follow up and responsive inspection in August 2012. We had issued a warning notice at the August inspection in respect of regulation 12 infection control, which the provider had complied with by the time we visited again in September. The August inspection had also highlighted some new compliance actions, which we assessed for improvement at this visit to the home today.

We spoke with four people that used the service, one visitor and the manager. We looked round the home and viewed documentation and records.

People told us that they were satisfied with their care support, that they wished to be at home rather than in a care home and that they would have liked more to occupy them. One person said, "I have lots of friends here." Another said, "I just wish I could get better." The visitor told us they thought their relative that used the service had been adequately cared for and supported.

We found that people were kempt and well presented. They were comfortable and relaxed. We saw that staff assisted people discreetly and respected their wishes.

During an inspection looking at part of the service

We did not speak with people directly about this outcome. We spoke with a newly appointed manager and with the operations manager. We also looked round the premises and particularly in communal bathrooms and toilets to make a decision about compliance with outcome 8, regulation 12 - cleanliness and infection control.

During an inspection looking at part of the service

We carried out the inspection to check on the improvements made since our inspection in May 2012. On that occasion we made a compliance action for outcome 8 'cleanliness and infection control'.

On this inspection we also responded to information we had received in relation to outcome 4 'care and welfare of people that used the service' and outcome 13 'staffing'.

During a routine inspection

We spoke with six people that used the service and they told us they were satisfied with the care they received. Two people told us they were happy living at Wyton Abbey. A third person told us they didn't have many people to talk with.

People wanted to tell us about their past lives, their conditions or health care needs and also about their immediate situation living at the home.

Everyone we spoke with presented as having high levels of need because of dementia conditions. We also spoke with two visiting family members who said they thought their relative was well cared for.

We observed people who demonstrated positive moods and satisfaction with the care and support they received.

In order to make a decision about compliance in most of the standards we carried out a Short Observational Framework for Inspection (SOFI), spoke with the manager and staff and checked documentation.

During an inspection in response to concerns

We carried out the inspection because we had received information from East Riding of Yorkshire Council that a high number of falls had been recorded in the home since September 2011. This information did not reflect the number of notifications we had received from the home manager.

We did not speak with people directly about any of the outcomes assessed at this inspection.
