The provider now had effective systems in place to ensure people’s capacity was assessed and Deprivation of Liberty Safeguards (DoLS) were applied for. People's needs continued to be assessed, and care and support was delivered in line with current best practice to achieve effective outcomes. People’s healthcare needs continued to be documented, and they were supported to access healthcare services. The provider continued to work effectively within and across organisations to deliver effective care, support and treatment.

Delivering evidence-based care and treatment

People benefitted from staff who were trained in current best practice. People and their relatives were involved in discussions about their care.

Staff delivered people’s care and support based on good practice standards and guidance. Staff monitored and recorded people’s health, such as their weight and food and fluid intake to ensure their nutrition and hydration needs were effectively met.

The provider’s processes helped to ensure people received care which was based on up-to-date guidance and legislation. People’s dietary requirements were assessed and monitored, and menus were changed to give more choice and diversity of food.

Supporting people to live healthier lives

Score: 3

People were encouraged to take part in activities and events to enhance their wellbeing. They had control over their own care and were involved in health checks with health and social care professionals, as appropriate.

Staff liaised with other health and social care professionals, such as GPs and dieticians where there were concerns about people’s health and well-being and followed their recommendations.

Processes were in place so people could access support from additional services, such as eye and hearing specialists to manage their health and wellbeing.

Monitoring and improving outcomes

The provider had processes in place which showed how people’s care was monitored and reviewed by staff, the registered manager and external health professionals. This helped to ensure people received coordinated approach to their care and had positive outcomes.

Staff regularly reviewed people’s care needs to ensure continuous improvements were made to their care, as and when needed.

People’s care was coordinated because other health professionals were involved. One relative spoke about their expectation of a clinical test for their family member and this had been organised by staff.
