There was not enough oversight from the provider or registered manager to ensure systems were in place and being used to provide good quality safe care. Checks and audits were not always being completed in important areas and were not robust enough to identify areas of shortfall found at this assessment. Staff told us they felt able to raise concerns and spoke positively about the registered manager.

Shared direction and culture

Staff were kind and caring and had shared views about how to meet people’s needs and provide inclusive care. Systems did not always support people to provide person-centred care due to inaccurate and sparse care planning records. We observed kind and caring exchanges between staff and people living at the service.

There was no strategic plan in place to drive improvements and improve the culture of the service. The registered manager wanted to make improvements at the service but there was no clear vision on how this would be achieved.

Capable, compassionate and inclusive leaders

The provider had no formal processes in place to monitor the performance and competency of the registered manager and offer support. The provider had not conducted any audits at the service since the registered manager had been appointed. Hyde Nursing Home has been rated requires improvement under this provider on two previous occasions. As a result, there has been closer input from the local authority and regular updates provided by the service. The leadership at the service has failed to drive sustainable improvements.

Staff gave positive feedback about the registered manager. Staff told us they felt supported by the registered manager and acknowledged their workload was a lot for one person. The registered manager told us they were committed to making improvements at the service but little progress had been made. There was no clear strategic plan to make this improvements.

There was signage around the home informing staff about having the freedom to speak up and everyone’s responsibilities.

Staff felt able to raise concerns with the management team. Staff told us they felt they would be listened to by the manager if they raised concerns.

Governance, management and sustainability

The service was not managed well. The service did not have effective governance systems in place to monitor and drive improvement, The provider and registered manager had poor oversight of the service. Audits completed at the service were not consistently robust and did not lead to effective action. Since September 2023 the occupancy of the service had increased from 40 to 71 people. During this time, the service did not conduct any care plan audits. The care plans were not person centred, inaccurate and lacked detail for staff to support people effectively. The service were not consistently responsive to feedback from other agencies. The local authority conducted their own care plan audits for a sample of residents in November 2023. Multiple actions were identified. At the time of the assessment, limited progress had been made. Following the recent concerns at the service, the local authority fed back that the service had began working more closely with them. Accidents and incidents were not being reviewed in a timely way to analyse trends and drive improvements. No accidents and incidents had been reviewed at the service since November 2023. The registered manager was aware of multiple areas where improvement was required. For example, moving and handling competency checks for staff, however there was no clear plan or timeline when these issues would be addressed. The management structure and responsibilities were not robust. There was a registered manager in place and a recently appointed deputy manager. It was not clear what responsibilities people had. The units did not have leaders on them so there was no one taking responsibility for daily monitoring. During the assessment, the registered manager informed us that unit managers were being appointed. The provider had not conducted any of their own audits since August 2023. There were no records available to demonstrate how the registered manager had been supported and monitored in their new role.

The registered manager told us they had not completed care plan audits at the service as these had been conducted by the local authority. Staff wanted the service to improve.
