How staff, teams and services work together

Score: 3

People told us staff and professionals work together to provide effective care and support. One person told us, “I have a social worker. They come and check on me.” We observed people meeting visiting professionals and having the opportunity to talk with them privately.

Staff told us they worked in partnership with other health and social care professionals. One staff member said, “We work closely with social workers, GP's and the pharmacy. I rang the GP this morning to discuss 1 person’s medicine concerns.” Another staff member told us how the home received regular visits from social workers and how they supported people to access their medical appointments outside of the home, such as doctors and hospital appointments.

Records confirmed partnership working. The provider raised concerns and shared relevant information with other professionals.

Visiting social care professionals told us the provider worked in partnership with them to promote the wellbeing of people living in the home. One visiting professional told us, "The management team have been really helpful and responsive. There is always someone around to answer questions. They have facilitated all my meetings."
