During a routine inspection

We used a number of different methods to help us understand the experiences of people who used the service. This was because the majority of the people who used the service had complex needs which meant they were not able to tell us their experiences. For example, we observed care practices and talked to people's relatives and staff. People who were able to tell us of their experience of living at The Dales told us they were generally happy living at the home.

We spoke with several visitors and they told us they were pleased with the standard of care provided by the service. One person told us their relatives had lived at the home for a number of years and they had always been satisfied with the care provided and were always made to feel welcome when they visited. Another person said 'The nursing staff always contact me if there are any significant changes in my relatives' physical or mental health, which I find very reassuring.'

We spoke with one visiting health care professional who told us they had no concerns about the care, support and treatment people received and that staff always followed their advice and guidance.

However, we found on two units the provider did not have effective systems in place to prevent, detect and control the spread of health care associated infection.

During an inspection looking at part of the service

We used a number of different methods to help us understand the experiences of people using the service. This was because some of the people using the service had complex needs which meant they were not able to tell us their experiences. We used the short observational framework for inspection (SOFI). SOFI is a specific way of observing care to help us understand the experience of people who could not talk with us. We were also supported on this inspection by an expert by experience. This is a person who has personal experience of using or caring for someone who uses this type of care service.

Three inspectors visited Ribblesdale, Wenslydale and Littondale units and the expert by experience visited Rosedale and Littondale. They engaged with nine people using the service, one relative, eight members of staff a acting home manager and a visiting health professional.

We carried out a SOFI on three units, on all of the units we observed that staff engaged with people in a respectful manner. They explained their actions and provided people with choices and gave them the time to make decisions. For example, when one person said they would like a glass of juice staff asked which flavour they would prefer, when biscuits were offered mid morning they asked people what type and how many. We saw two people, who were upset, were offered the opportunity to go for a walk to help distract them. One relative also confirmed that people get choices all the time, especially at mealtimes. Another relative told us one day their relative really fancied chips, staff went to speak to the cook and they made them chips for tea.

During a routine inspection

Two inspectors visited the service and used a number of different methods to help us understand the experiences of people using the service, because some of the people using the service had complex needs which meant they were not able to tell us their experiences. So we sat in the lounges on Littondale and Ribblesdale units and observed people's experiences for over three hours and talked with three people using the service, two relatives, and two health professionals. We also talked with the temporary manager, the quality consultant and five members of staff.

People provided us with different responses when we asked them their experience of living at The Dales. One person told us the staff were kind and helpful. A relative told us their relative was 'Much better here than at the last home they were in'. However another person said they 'Hated it' and a further person said it 'was alright'.

One person told us they did not feel safe and a relative told us they were concerned about their relative's safety in the home.

We observed the interactions between staff and people living in the home on two units. On one unit we observed the staff were friendly and kind and constantly offered help and support to people. Whilst on another unit we observed staff did not always respond to peoples needs promptly.

We also observed how peoples care and welfare needs were not always met fully and where staff had failed to identify where people may have been at risk
