Improvements had been made in people’s care records to evidence care provided to people and action taken to respond to concerns identified. Staff knew about people’s needs and assessments and care plans were reviewed and updated regularly. However the management team and staff were still working on improving these. During the site visit we observed that people mostly received care and support in accordance with their assessed needs. However there were area’s which required improvement.

Assessing needs

At the last inspection, we found the provider in breach of Regulation 9 (Person centred care) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.This was because the provider had not always ensured that the care and treatment of people was appropriate, met their needs or reflected their preferences. At this assessment we found that improvement had been made and the provider was no longer in breach of Regulation 9. Care plans contained clear guidance for staff around any health needs. For example, we looked at the plan for one person, for whom sufficient fluid intake was an important part of their care. Although the plan informed staff of a recommended daily fluid intake, records showed this target was not always met and there was nothing documented to show that staff had noted this or escalated the poor fluid intake. We discussed this with the manager who told us staff did monitor people’s fluid intake and would take action if needed; however, there were no records to show this was done. The manager told us they would ensure staff started to document this going forward.

Staff knew about people’s needs and assessments and care plans were reviewed and updated regularly. Staff comments included: “We know about each person's needs as we have to read their care plan and we also have a hand over when our shift starts. We also get to know people over time; we get to know their routines and their likes and dislikes" and “In my senior role, I update care plans and risk assessments. I do like the new system but the internet here isn't that great and sometimes recording is a problem.”

People had their needs assessed and received care and support in accordance with their assessed needs. During the site visit we observed that people mostly received care and support in accordance with their assessed needs. However, we noted that in the lounge area people all received drinks in plastic cups. This was not person centred. We discussed this with the provider who gave assurances this would be addressed. Improvements had been made by the second day of our visit.
