During an inspection looking at part of the service

We carried out an unannounced focused inspection at Oaktree Clinic Midlands in response to concerns raised about the quality of care provided. As it was a focused inspection, we only inspected parts of two of the five key questions which were Safe and Well led. This service has not previously been rated.

How we inspected this service

Before visiting, we reviewed a range of information we hold about the service. Prior to the inspection we reviewed any notifications received, and the information provided from pre-inspection information requests.

During our visit we:

Spoke with 6 members of staff at the service;

Looked at the equipment and rooms used by the service;

Reviewed twelve case records, one personnel file, clinical policies, minutes of meetings and other policies;

Checked the governance processes in place for the storage, use and destruction of prescriptions;

Checked clinic equipment had been calibrated and PAT tested;

Looked at two employee records;

Spoke with the registered manager, clinic manager and other staff employed by the service;

and reviewed information and documents relating to the operation and management of the service.

The areas where the provider should make improvements are:

• The provider should ensure staff complete appropriate risk assessments for patients prior to the use of Transcranial Magnetic Simulation (TMS) treatment.

• The provider should ensure staff know how to report safeguarding concerns.

• The provider should ensure staff understand the vision, values and strategy and their role in achieving them

• The provider should ensure all staff receive appraisals in line with provider policy

• The provider should ensure staff have access to regular team meetings

• The provider should ensure that all clinic equipment is maintained to ensure it is ready to use when required

• The provider should consider implementing a whistle blowing policy for staff to be able to raise any complaints or concerns they may have.

• The provider should consider providing a freedom to speak up guardian, so staff have an independent person to speak to raise concerns
