During an inspection looking at part of the service

At the last inspection in October 2013 we found that people who used the service and their relatives were happy with the quality of care provided. However, in part of the service there was a lack of up to date and detailed information about people's needs. This meant people were at risk of experiencing support that did not fully meet their needs safely and effectively. We also found that the provider's system to monitor the quality of the service was not effective.

We asked the provider to make improvements to ensure the required standards were met. On this follow up visit we found that people's needs had been reviewed and reassessed and that care plans and risk assessments had been updated. Staff now had all the information required to ensure people received care and support safely and in line with their needs.

The provider had carried out a 'support audit' for the service during September and October 2013. This identified areas that required improvement and included the concerns found in our previous inspection. In addition more robust systems had been put in place to ensure that the quality of service was monitored and that people received an effective and safe service which met their needs.

During a routine inspection

Overall people who used the service and their relatives were happy with the quality of care that they received. A person who used the service said 'I like them, they help me.' A relative told us 'we as a family are very, very happy. They are absolutely fantastic and are always available to talk over things.' However, in part of the service people's needs were not always assessed and care and treatment was not always planned and delivered and in line with their individual care plan.

We found that people were treated with respect and that they were asked for their consent before any care was provided. We saw that the provider worked in co-operation with others to ensure that people's health, safety and welfare were protected.

Some people received support from staff to administer their medication. The provider had systems in place to train staff and ensure that people received their prescribed medicines safely and appropriately.

We found that people were supported by a regular staff team who had the skills and experience to meet their needs. One relative told us "they have definitely got the skills to do the job."

Although the provider had quality monitoring systems in place these were not always effective. Action to improve part of the service, identified in an audit in June 2012 had not been completed and the provider had not identified that this was still outstanding.

During a routine inspection

The service provided one to one outreach support and this was mainly supporting people with activities of their choice. In addition the service is registered with the Care Quality Commission to provide personal care. At the time of this inspection only two people were supported with this. We spoke to one person who received support with personal care and also to one person who used the outreach service but did not receive support with personal care. Those we spoke with were happy with the quality of the service and support that they received. We also saw some responses from a quality feedback surveys and these were positive.

A person who used the service told us, 'They are all very nice ladies and I like them. They always ask me what I want to do and they treat me very very well. They treat me respectfully and I feel like they're my friends. My mum and dad are happy with the service as well. Another person said, 'They are brilliant, they never let me down and always treat me okay.'

In response to a quality feedback survey people had written: 'Flexible reliable service, professionally managed, pleasant and polite staff. Also, 'People care and are nice and helpful and friendly.'

We spoke with two of the outreach staff who were in the office on the day of the inspection. One said, 'People get to go out where they want to go. They try new things and they're happy. We provide very little personal care support. For one person we prompt them to do things and explain how important is to do these things and encourage them to do what is needed.'
