Delivering evidence-based care and treatment

Most people felt the food was nice and told us they were offered choice. One person though told us staff did not always prepare food in the consistency they required it. People and relatives told us that people did not always receive regular drinks at the service. The mealtime experience was disorganised and some people were waiting long periods for their food to arrive. One person told us that they often were not supported to bathe. A relative told us staff did not always support their loved one appropriately with their catheter, and this had caused difficulties at times.

Not all staff demonstrated a confident understanding of people’s needs and best practice. Staff told us they knew about the requirement to complete records in relation to people’s food and fluid intake, when this was needed to monitor their health and wellbeing. However, their recording and practice in this area was poor, which showed a lack of understanding.

Food and fluid charts were not consistently completed, which meant it was not possible to effectively monitor that people had had enough to eat and drink and to take appropriate action to reduce any risks to people. People’s fluid intake targets were not always met, and there was limited evidence about what action had been taken to encourage people to drink more when needed. This meant people were at risk of becoming dehydrated. There was also a lack of evidence to show staff had recognised and responded promptly when people were showing potential signs of constipation. We identified a breach of regulation in relation to nutrition and hydration.
