People received care in accordance with their assessed needs. Care records and risk assessments were amended when people’s needs changed. This ensured people continued to receive care that met their needs.

Assessing needs

Score: 3

People were involved in decisions about their assessed needs. Where appropriate, relatives were welcomed to meetings to discuss their family member’s needs and changes to care. Many relatives praised the admission process to the home. They welcomed the supportive nature of the management and staff and welcomed being involved with the initial decisions about how the care was to provide. One relative stated they felt “reassured” by this approach.

Staff felt they were provided with sufficient information via care plans and other relevant formats to enable them to provide care in accordance with people’s assessed needs. The provider had ensured that the registered manager received the appropriate support needed to ensure care records were reviewed regularly to ensure they continued to meet people’s assessed needs. The registered manager confirmed this.

People’s care needs were routinely reviewed, and risk assessments were up to date. People’s needs were assessed using a range of assessment tools to ensure their needs were reflected and understood. This included assessing the risk of people developing pressure sores and if people were at risk of dehydration and losing weight. The provider ensured assessments considered people’s health, care, wellbeing, and communication needs, to enable them to receive care and treatment that had the best possible outcomes. External agencies were referred to should people require more in-depth treatment. This included referrals to tissue viability nurses, dietitians, and GPs.
