People were not always involved in decisions about their care, there was no evidence people were involved in the assessment process and reasonable adjustments were not always carried out to meet the needs of people in the home. There was a culture of ‘blanket restrictions,’ and minimal understanding of person-centred care. This was a breach of regulation 9 of the Health and Social Care Act 2008 (Regulated Activities) 2014.

Person-centred Care

All relatives we spoke with told us they were generally happy with the care people received; however, they would like them [People] to be out of bed more and assessed for other things to improve their lives. People and their relatives told us they had never seen a care plan or had been involved in reviewing care plans.

Professionals told us care plans and risk assessments were not always followed and were not individualised to the person. Recommendations from professionals were not written into care plans and risk assessments to support individualised person-centred care.

Peoples care plans did not fully reflect their needs. Some care plans and risk assessments had not been updated since people’s admission assessment. There was no evidence anyone had been involved in the assessment process including families for those people who did not have capacity to make specific decisions. We saw no evidence reasonable adjustments had been made to support and improve people’s quality of life.

People were not involved in planning their care and support. People told us they had never seen a care plan or been involved in writing one.

Staff told us everyone is treat the same but were unable to give any examples of someone who was likely to experience inequality and how they would care for them.

People who it was deemed did not have capacity were not assessed or had a best interest meeting in place to ensure care and support was tailored to them and chosen by them.
