On and/or before 29 May 2024, the employer was found in breach of regulations 6(5)(b), 8(1), 14(2), 15(1)(b) and 17(4).
Description
An Improvement Notice was issued to the employer under Section 21 of the Health and Safety at Work etc. Act 1974 following an inspection of the radiotherapy department. This identified concerns around quality assurance programmes for written procedures and protocols, clinically significant unintended or accidental exposure procedures, medical physics expert provision, the equipment inventory and records of staff training.
Result
The employer submitted evidence and was deemed to comply with this notice on 02 September 2024.
