Breach
On and/or before 23 January 2024, the employer was found in breach of regulations 8(4) and 11(5).
Description
An Improvement Notice was issued to the employer under Section 21 of the Health and Safety at Work etc. Act 1974 a comprehensive proactive inspection of the radiotherapy department. This identified concerns around the management, investigation and recording of incidents (8[4]), and guidelines regarding the authorization of exposures by operators (11[5]).
Result
The employer submitted evidence and was deemed to comply with this Notice on 11 March 2024.
