Staff do not always recognise when people need or want help and support from family, friends or others. Where they do, they may not do so in culturally or personally sensitive ways, or recognise when people's preferences are not being taken on board, or properly respected.
The service may not have gathered information about available sources of information, advice and advocacy to help people and their families, or it does not always pass information on when it could help.
Staff do not always involve people and, as a result, they do not feel listened to.
