This review looks at how well hospitals investigate when care has gone seriously wrong, and whether they are learning from these cases.
We will examine how the use of these investigations can reduce the amount of 'avoidable' harm to people. This work will feed into the development a separate body the government is setting up to investigate this kind of case and help CQC make better use of information about how hospitals respond to serious incidents.
