Dr SKS Swedan and Partner based at the Lord Lister Health Centre in the London borough of Newham, has been rated Inadequate overall by the Care Quality Commission.
It was rated Inadequate for being safe and well-led and Requires Improvement for being effective caring and responsive, following the inspection in January 2020. The provider has now decided to cease operating and has cancelled its registration with CQC.
At this inspection insufficient improvements had been made and the practice was rated inadequate overall for the third time in four years. The provider took immediate steps to cancel its registration with the CQC and will no longer provide services after Tuesday 31 March 2020.
Inspectors found that patients’ test results had been checked by a non-clinical staff member that was not qualified or assessed as competent to do so.
The practice did not have clear systems and processes to keep patients safe including in the event of a medical emergency.
Patients prescribed high risk medicines were not appropriately monitored.
There was a lack of awareness and action to address areas of ongoing underperformance and risk including patients’ satisfaction, safety, and clinical care.
The leadership team did not consistently demonstrate they had the experience, capacity and capability to run the practice and ensure high quality care.
There were divisions within the leadership team that had prevented action to address essential improvements that were required. The overall governance arrangements were ineffective.
Head of Inspection for Primary Medical Services in London, Antony Hall, said: “It is always disappointing when a GP practice consistently performs as badly as this practice.
“Over a four-year period, the practice was rated Inadequate overall on three occasions and CQC was about to cancel its registration before the provider decided to do so itself.
“I would like to reassure patients that after 31 March, when the present provider will cease providing services, that they will be looked after by a caretaker practice.”
