Use this form if you want to apply to register as a new provider with CQC

If you have more than one location, nominated individual or two partners

The provider application form contains space for details of one location, one nominated individual and two partners.

If you need to include more details, complete the relevant form for each addition as well as the main form.

Additional locations

If your application includes more than one nominated location, fill in this form for each additional location. 

Additional partners

If your application includes more than two partners, fill in this form for each additional partner.

Additional nominated individual

If your application includes more than one nominated individual, fill in this form for each additional nominated individual.

Services for people with a learning disability or autism

If you're applying to provide a residential service for people with a learning disability or autism check you meet the requirements: Right support, right care, right culture.

You will then need to complete an additional form with your application. This will show how you will meet the requirements for these services.

Email your completed application to us

Email your completed form and supporting documents to HSCA_Applications@cqc.org.uk.

The largest email size we can receive is 10MB. If your email is larger than this when you have attached the necessary documents, you should split it into more than one email.

You should include the following details in your email subject line:

  • provider name
  • application type (in this case, new provider application)
  • the number of emails you are sending as part of your application.

Example:

  • Oxtown Care Limited new provider application 1/2
  • Oxtown Care Limited new provider application 2/2.
