When and how to tell us about the death of a detained mental health patient.

When you need to tell us about a death

You must notify us when:

  • a person has died while being detained (or liable to be detained) under the Mental Health Act 1983

You must notify us of the death as soon as possible.

If the death is related to a patient safety incident, you should also report to your local risk management system (LRMS). Use the LRMS report number when you notify us so we can identify the duplicate reports.

You must use this up-to-date form and provide all the information we ask for. If your form is incomplete we will return it to you.

Email the completed form to MH_notifications@cqc.org.uk.

Your notification

Your acknowledgement number may be different to what you expect. As we upgrade our systems you will receive either an ENQ reference number, notification ID, or case ID. Any of these numbers will identify your notification.
