If you are a registered manager of health or adult social care services, there are several types of application you can make to us. These allow you to make changes to your registration status.
Continue my registration under a new provider
If you are an existing manager and your location is taken over by a new provider, you can change your provider on your registration using this shorter form and simpler process.
Add a regulated activity
Use this form if you want to manage a new activity regulated by us. You will need to submit one copy for each regulated activity being added. This form will also allow you to add new locations at the same time.
Remove a regulated activity
Use this form if you will no longer be managing an activity regulated by us. You will need to submit one copy for each regulated activity being removed. You cannot use this form to cancel your whole registration.
Cancel your CQC registration
Use this form if you no longer intend to manage any activities regulated by us. If you are going to be managing for a new provider at a different location, you must use this form to cancel your existing registration (and then apply for a new registration).
Add a location
Use this form if you want to manage regulated activities already included in your registration at a new location. For example, if you want to manage activities at a new hospital, care home or surgery.
Remove a location
Use this form if you no longer intend to manage activities regulated by us at one of the locations included in your registration details.
Vary or remove a condition
Use this form if you want to have the limitations of a condition changed or lifted. This does not include conditions on where you manage activities - to change these you must apply to add or remove locations.
Vary or cancel a suspension of registration
Use this form if you have had your registration suspended and you wish to begin managing activities again before the suspension ends.
