You must provide all the information we ask for. If your form is incomplete we will return it to you.

Plans for the service
Plans for the future of the service should be made as soon as possible to ensure that the regulated activity is not being carried on by an unregistered person. This would be an offence.
Individual providers
If an individual provider dies, as well as notifying CQC of their death, the individual’s personal representative must also tell us about their plans for the service. This must be done within 28 days of the death of the registered provider.
Partnerships
Where the death of a partner leaves only one surviving partner, they should tell us about their plans for the service using the additional form: plans for a service following the death of a registered provider. This form includes a field for the name of the partnership.
The surviving partner may need to apply to cancel the registration of the partnership.
- If they sell or transfer the service to another provider, they must submit a cancellation application when the new provider applies to register or add the location if they are already registered.
- If they are continuing to provide the regulated activity themself, they may need to cancel the original partnership registration and apply to register as an individual, an organisation, or a new partnership. The surviving partner may want to seek legal advice on what happens to the partnership following the death of their partner.
