A Newham practice has been rated Inadequate overall by the Care Quality Commission, which is a deterioration from its previous rating.
Dr Tun Lwin’s practice in Prince Regent Lane, is now rated Inadequate for being safe effective, responsive and well-led. It was rated Good for being caring, following the inspection in August 2018.
Inspectors found people undertaking clinical work had not been subject to the necessary recruitment checks.
Reviews of patient records showed follow ups did not happen in a timely manner.
The practice did not have adequate systems in place to respond to medical emergencies. Care planning was inconsistent.
Complaints were not dealt with in line with practice policy and information on how to make a complaint was not easily accessible.
Areas where the practice must now make improvements include:
- Ensuring the care and treatment of patients is only provided with the consent of the relevant person.
- Ensuring care and treatment is provided to patients in a safe way.
- Establish effective systems and processes to ensure good governance in accordance with the fundamental standards of care.
- Ensure the correct number of qualified staff are on duty to meet accepted standards of care and treatment.
Areas where the trust should improve include:
- Improve systems for safeguarding - particularly related to chaperoning and training.
- Improve systems to ensure care plans are drafted where appropriate.
- Improve the practice’s complaints programme.
Professor Ursula Gallagher, Deputy Chief Inspector, Primary Medical Services, said: “I am very concerned that Dr Tun Lwin’s practice is now rated Inadequate overall and must be placed in special measures. At its last inspection it was rated Requires Improvement - with three categories rated Good.
“We have now placed the practice in special measures which will give it access to support that should help it to improve.
“People who use the service should be reassured that the service will be kept under review and if needed could be escalated to urgent enforcement action. Where necessary, another inspection will be conducted within six months, and if there is not enough improvement we will move to close the service by adopting our proposal to remove this location or cancel the provider’s registration.”
