The oversight systems related to the safe governance of the environment had not been sufficient. This was acknowledged in discussions related to fire safety, legionella and environmental risks. The registered manager and provider were responsive and took action during our assessment but we could not assess the sustainability of their actions. During our assessment of this key question, we found concerns around the governance and management of the service which resulted in a breach of Regulation 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. Governance, management and sustainability

Staff spoke positively about the registered manager and told us they felt supported. One staff member said, "This is a supporting place to work and is family orientated." The registered manager acted upon the feedback we provided throughout the inspection and immediately put actions in place to improve however, we were unable to assess the sustainability of these changes at this inspection.

There was a lack of robust systems and processes to identify areas where improvements were needed to ensure people were safe from their environment at Park House and risks to people had not always been identified and mitigated. The legionella policy was in line with current guidance and had been reviewed in September 2023. However, it had not been identified that the Park House water management processes was not in line with their own policy. There was no schematics of the premises water system indicating all dead legs had been removed and had no specific records of temperatures and flushing and the staff member named as the responsible person had not received suitable training. Whilst we found no evidence of harm, people had been placed at risk due to a lack of robust governance and oversight. This was a breach of regulation 17 of the Health and Social Care Act.
