Effective
The practice had systems and processes to keep clinicians up to date with current evidence-based practice. We found that patients’ immediate and ongoing needs were fully assessed these included, interventions required to ensure effective care was delivered. Staff received proper training and support for their work.

Assessing needs
Staff and leaders told us people’s needs were fully assessed before they could be accepted by the clinic. People had to provide comprehensive information about their medical history in their new patient application and this was fully verified by the consultants in their initial consultations. Leaders told us once people had been accepted their care would be reviewed every three months as part of their internal shared care arrangements.
The review of the patient record showed that patients received appropriate reviews with the shared care team and an annual review by a consultant. Patients who were identified as vulnerable received more regular consultant reviews.
Delivering evidence-based care and treatment
All consultants had to complete an external and internal CBPM training course, followed by shadowing a colleague before they could carry out a consultation. There was a system and a named manager who oversaw the various audits to support safe care and treatment.
How staff, teams and services work together
Staff told us they had access to the information they need to appropriately assess, plan and deliver people’s care, treatment and support.
The clinic had a ‘shared care’ protocol in place to ensure all relevant staff, teams and services were involved in assessing, planning and delivering people's care and treatment and staff work collaboratively to understand and meet people's needs.
