Vary or remove a condition
Use this form if you want to have the limitations of a condition changed or lifted. This does not include conditions on where you provide activities we regulate - to change these you must apply to add or remove locations.
Provide services from a new location
Use this form if you want to provide activities we regulate already included in your registration from a new location. For example, if you are opening a new hospital, care home, surgery or any other site where services are provided from.
Stop providing services from a location
Use this form if you no longer intend to provide activities we regulate from one of the locations included in your registration.
Make an administrative change to the address of any of your locations
Use this form if there is an administrative change in the address and there is no other change to the location or the facilities provided to people who use services. For example the name of the building might have changed.
Change the address of any of your locations due to expansion
Use this form if you are expanding the address of a location. For example your address might have changed from 1-2 High Street to 1-4 High Street. You will need to explain the changes and the impact on people who use services. We may need to visit the new address to assess the application.
Change your business and location address (where these are the same)
Use these forms if you want to completely change your business and location address. Your business address must be the same as your location address.
Vary or cancel a suspension of registration
Use this form if you have had your registration suspended and you want to begin providing activities we regulate again before the suspension ends.
