Breach

On and before 10 September 2020, the employer was found in breach of regulation(s) 6(1), 6(2), 6(5)(a), 6(5)(b), 8(4), 14(3)(b), 14(3)(i) and 17(4).

Description

An Improvement Notice was issued to the employer under Section 21 of the Health and Safety at Work etc. Act 1974 following announced inspections that were carried out on 8 & 9 September 2021. This identified the following concerns; poor quality of documents relating to IR(ME)R, poor control and dissemination of new procedures to staff, lack of referral guidelines, poor understanding of notification requirements relating to accidental and unintended exposure that resulted in incidents not being notified, insufficient support from the Medical Physics Experts that resulted in an inadequate quality assurance programme and insufficient advice regarding regulatory compliance and training records were not available at the time of inspection.
